By using gamedcard.com or pre-registering for a consultation with Dr. Rasean Hodge, you agree to these Terms.
What this service is
We help Georgia residents pre-register for a free 15-minute consultation to see if they qualify for the state's Low THC Oil Registry under SB 220. A physician-patient relationship is only formed once Dr. Hodge sees you at your scheduled consultation.

Consent to be contacted (TCPA)
By submitting the pre-registration form, you consent to be contacted by phone, email, or SMS about your consultation and Low THC Registry application. Message and data rates may apply. Reply STOP to any text to opt out of further texts. Opting out will not affect our ability to reach you about a scheduled appointment.
Fees
The initial consultation is free. If Dr. Hodge determines you qualify and you elect to proceed, the current fee for the physician certification is $149.99, or $124.99 with promo code PLANTS25. Payment is due at the time of certification and is non-refundable once submitted to the Georgia DPH.
Eligibility
You must be a Georgia resident and at least 18 years old (or accompanied by a legal guardian) to receive services. Photo ID and proof of Georgia residency are required.
Accuracy of your information
You agree that information you submit is true and accurate to the best of your knowledge. Submitting false information to a state registry may be a crime.
Limitation of liability
To the fullest extent permitted by law, Plants Over Pills and Rasean Hodge, MD are not liable for indirect, incidental, or consequential damages arising out of your use of this site. Nothing in these Terms limits liability that cannot be excluded under applicable law.
Changes
We may update these Terms. The "Last updated" date at the top reflects the latest revision. Continued use after changes constitutes acceptance.
