HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment

Rasean Hodge, MD (dba Plants Over Pills) is required by federal law (HIPAA) to maintain the privacy of your Protected Health Information ("PHI"), provide you with this Notice, and follow the terms of the Notice currently in effect.

How we may use PHI

• Treatment: to evaluate you, prepare a Low THC Oil Registry certification, and coordinate care.
• Payment: to bill you for the medical card fee if you elect to move forward.
• Health-care operations: quality review, appointment reminders, internal training.
• Required by law: reporting to the Georgia Department of Public Health for the Low THC Registry when you authorize submission of your application.

When we need your written authorization

We will obtain your written authorization before using or disclosing PHI for marketing, selling PHI, or most disclosures of psychotherapy notes. You can revoke your authorization at any time in writing.

Your rights

• Access and receive a copy of your PHI.
• Request corrections to your PHI.
• Request an accounting of disclosures.
• Request confidential communications (e.g. call cell only, no texts).
• Request restrictions on how we use or share your PHI.
• File a complaint if you feel your privacy rights have been violated — with us, or with the U.S. Department of Health & Human Services Office for Civil Rights. We will not retaliate.
