Overview
Georgia's registry originated to serve patients with intractable seizures, and seizure disorders remain one of the broadest qualifying categories. Both adults and pediatric patients qualify with a treating physician's diagnosis.
What the evidence says
CBD is FDA-approved (Epidiolex) for Dravet, Lennox–Gastaut, and tuberous sclerosis. Low-THC/high-CBD preparations are widely used adjunctively for refractory epilepsy.

- Neurology notes with seizure type and frequency
- List of anti-epileptic drugs tried
- For pediatric patients: parent/guardian ID and proof of guardianship
- Front & back of your Georgia driver's license or state ID
